Care Sourcing

This tailored service provides practical support to help you find the right care, whether that is a care home or support at home. Following an initial consultation, we will work with you to understand what is most important, your preferences, and your circumstances. We will provide clear advice on what options are available, research suitable providers, and share a clear, easy to understand shortlist with key details on each.

This service is designed for self-funders who want independent guidance and support when making important care decisions. Where appropriate, we can also help you understand typical costs and ensure you are getting a fair and reasonable price, helping to reduce stress and allowing you to move forward with confidence.

Continuing Healthcare (CHC) Checklist Support

This service supports you in understanding and preparing for the first stage of the NHS Continuing Healthcare assessment process. Whether a checklist has already been completed and you would like help understanding the outcome, or you are unsure where to start with requesting CHC and asking a practitioner to complete a checklist, we can guide you through the process.

We will review your situation together, explain what assessors are looking for, and help you identify key areas of need. The aim is to ensure you feel informed, prepared, and more confident at this early stage.

Continuing Healthcare (CHC) Decision Support Tool (DST) Support

This service is designed to help you prepare for the NHS Continuing Healthcare Decision Support Tool (DST) assessment. Whether you have been told a DST will be completed or you are approaching a multidisciplinary meeting and want to feel more prepared, this session will guide you through what to expect.

We will work through the document together, explaining each domain, what assessors are looking for, and how this applies to your individual situation. This helps you feel clearer, more informed, and confident going into the assessment, with a better understanding of how decisions are made.
